US expert surgeon supports reflux medical method for gastroesophageal reflux disease (GERD) and Barrett‘s esophagus

Here Reflux medical reports on a scientific international meeting on Gastroesophageal reflux disease ( GERD) and Barrett‘s esophagus (BE) held in Salzburg Dec 10th - 11th, 2015.
During the meeting the well known, highly reputed US expert surgeon Prof. Dr. Tom DeMeester, Los Angeles, CA, summarized the novel developments regarding GERD and Barrett‘s esophagus. Going in line with the novel understanding

  • GERD is a progressive disease,
  • presents with a large spectrum of symptoms,
  • may progress to cancer and
  •  should be defined by histopathology.

Furthermore, the therapy of GERD and Barrett‘ s esophagus should be based on histopathology. Thus Barrett‘s tissue with increased cancer risk should be eliminated using endoscopic resection and / or radiofrequency ablation (RFA). RFA is a novel endoscopic method for effective eradication of Barrett‘s tissue and durable cancer prevention (see RFA on our web site). Anti reflux surgery should be considered, when Esophageal manometry and Reflux monitoring have demonstrated, that abnormal Reflux is the reason for the symptoms. The modern spectrum of Anti reflux surgery includes endoscopic (stretta, endoscopic Fundoplication) and laparoscopic techniques ( Fundoplication, LINX Reflux management system, endostim). Following adequate patient selection the short and long term success rates after Anti reflux surgery equal 85%-95%. In order to achieve these excellent results diagnosis and therapy should be conducted in specialized centers with respective experience in diagnosis and therapy of GERD and Barrett‘s esophagus.

Taken together the presentation of Prof. Dr. Tom DeMeester supports the approach of Reflux medical, i.e. histopathology based diagnosis and treatment of GERD and Barrett‘s esophagus; the first line treatment of GERD includes medical (proton pump inhibitor; PPI to block gastric acid secretion) and nutrition therapy (reduce concentrated sugar consumption); Anti reflux surgery is offered only for abnormal Reflux - positive individuals.
